Dear FOIA Officer,

I am writing to request information under the Freedom of Information Act (5 U.S.C. 552). Specifically, I am seeking copies of the last five completed final reports on chemical accidents and safety incidents that have been investigated and concluded by the U.S. Chemical Safety and Hazard Investigation Board (CSB).

My request encompasses:

  • Final reports of the five most recently completed investigations into chemical accidents and safety incidents, including all findings, conclusions, and any appendices or supplementary materials.
  • If available, any executive summaries or public releases associated with these reports.

This request is made in the public interest, to understand and disseminate information regarding chemical safety and the prevention of future incidents. Please inform me of any potential fees for searching or copying these records in advance. However, I would like to request a waiver of all fees, as the disclosure of this information is in the public interest and will contribute significantly to public understanding of health, safety, and environmental risks associated with chemical operations.
